住所変更 Change of address

• 移転した日から14日以内に届出してください。（市役所1階3番窓口）
  Changes of address must be completed within 14 days of the change at window #3 of the 1st floor of City Hall.
  Please report within 14 days from the date of relocation (City Hall 1st floor,3rd window)
• 届出時に必要なものは以下のとおりです。
  The items required for the notification are as follows.

山形市内への転入 Moving in to Yamagata City

• 在留カード
  Resident Card
• 転出証明書（移転前の市町村役所からもらいます）
  Moving out certification（from previous address’s municipal office）
• 年金手帳（国民年金加入の方）
  Pension Book（Subscriber）
• 個人番号カード（異動者全員分）
   Individual number card（For all transferees)
